Gameplay
The story begins as the playable character, Billy West, realizes that he must fight to save his girlfriend Annabelle Lane from the gang of Godfather Gordon. Along the way are six side-scrolling fighting stages, two light gun shooting stages, and two driving stages.
In the fighting stages Billy can pick up and use many weapons including a gun, he also has punching, kicking and jump kick attacks. The shooting strages can be done with the controller or the zapper light gun. They are in a first-person perspective and ammo must be wisely conserved so that enough is left for the boss at the end. Health and bullets can be picked up in the stage and an hourglass power-up gives unlimited ammo for a short time. In the driving stages the player controls a jeep through the swamp to the city, manuvering past obstacles and enemy vehicles. Billy's jeep is equipped with grenades and a machine gun to take out enemies and gasoline can be picked up to gain additional time. Each type of stage also has a practice mode. It is wise to use these practice levels because the player gains a power-up that can be used during the main game after completing a practice level. In the driving level you get a tank of fuel. In the shooting level you get more bullets. In the fighting mode you get more meat for health.
Regional differences
The differences in the original Japanese version, entitled Mad City, was that it was not nearly as difficult as the international versions. For its worldwide release the enemies for the most part are faster and stronger plus the player has less bullets in the shooting stages. Also changed was the girlfriend Annabelle from wearing a red dress to denim shorts and a red shirt and the ability to end your relationship with her after the final boss. The international version also added DPCM-coded voice samples, being one of the few NES games to do so. A male voice boomed the title of the game during the game's opening screen and says "bye" to the player at the end of the game. Also, the villain of the game, Godfather Gordon, ends all his taunts to Billy with a smug laugh between stages.

Comic
Archie Comics published a comic book adaptation of The Adventures of Bayou Billy written by Rich Margopoulos and illustrated by Amanda Conner. The comic lasted five bi-monthly issues, dated from September 1989 to June 1990. It featured some of characters from the game, as well as original ones. Billy was made a widower (as his first wife was murdered), and Annabelle's surname was changed from Lane to Lee.
Television
Bayou Billy also received some amount of exposure on the Nintendo-themed animated TV series, Captain N: The Game Master, where Bayou Billy (modeled somewhat like Crocodile Dundee) appears in an episode titled "How's Bayou", playing on the difficulty of its real world counterpart, Bayou Billy was said to be the one game even Captain N was unable to conquer.
